Kingdom hearts synthesis materials list?

I was about to go and synthesize the ultima weapon until I realized I had to synthesize every item on the synthesis list before I can get the ultima weapon. I'm having trouble finding all the items for synthesis. Can someone give me a list of where to find some of these items?!
